Chief see's that McCarthy has lost all that he was before the lobotomy and decides to put him out of his misery and suffocate him, after chief does this, he finally leaves the institution. In the novel this shows that Nurse Ratchet lost all control over the men, this is a positive conflict, in the context of McCarthy sacrificing himself to save the others form Nurse Ratchet's control.
